Please complete the form below to register your fan(s), then click the "Register" button to provide your contact information.​

If you have only one fan, then complete the form only once. If you have 2-5 fans, it is recommended that you complete the form once per model number.

You may register up to 5 affected fans in total. If you have more than 5 affected fans to register, please call the recall hotline at 888-232-9895, Monday through Friday 8 AM CT to 5 PM CT.

When you are finished completing the form(s), and the item card(s) are correct, click the "Register" button at the bottom of the page.

Form Instructions:

Step 1 - select your the appropriate model number, quantity, and store from which you purchased the fan(s) *

Step 2 - click "Add Item"

​

Step 3 - review the item card for accuracy; click "DEL" to delete any inaccurate item cards

​

Step 4 - repeat steps 1-3 as needed

Step 5 - click the "Register" button to proceed

​

* If you have 2+ fans of the same model number but they were purchased at different stores, complete the form multiple times. It's possible to complete the form more than once for a given model number.

Do not dispose of your fan(s) at this time. You'll need to take photo(s) of them in the next step, photo validation. After you've confirmed your fan's model number is affected by the recall, consider reviewing the fan removal guide for information on how to safely uninstall your fan(s).
